A Health Insurance Option for Uninsured Hoosiers
Healthy Indiana Plan (HIP) is a State insurance plan that covers:
- inpatient hospital
- outpatient hospital
- physician services
- preventive services
- prescriptions
- diagnostic exams
- home health services
- hospice
- family planning
- case and disease management
- mental health
To be eligible for HIP, applicants must meet strict requirements:
- Age 18 - 64 (parents or caretaker relatives of children in the Hoosier Healthwise program are likely candidates for HIP)
- Annual household income up to 200 percent of the Federal Poverty Level
- Not eligible for Hoosier Healthwise or Medicaid
- Not pregnant (pregnancy is covered by Hoosier Healthwise)
- Ineligible for health insurance through an employer
- Uninsured for at least six months
HIP provides people with a basic commercial benefits package once annual medical costs exceed $1,100. It is built around a Personal Wellness and Responsibility (POWER) Account valued at $1,100 per adult to pay for initial medical costs. This is similar to a health savings account (HSA) and is used to fund the deductible.
HIP requires individuals to make mandatory monthly contributions - ranging from two percent to five percent of income, up to $92/month - to their POWER Account. To prevent participants from obtaining temporary coverage, penalties are stiff for payment lapses. Participants have up to 60 days to make their contribution and are then terminated and cannot reapply for 12 months if a payment is missed.
After their monthly contribution, participants have no other cost-sharing requirement except for copays for non-emergency usage of the emergency room. While contributions are higher than traditional Medicaid premiums, participants have total control over how these dollars are spent. Members receive monthly statements detailing the use of the accounts and can apply year-end balances to offset future required contributions if they have received requisite preventive services.

Step-by-Step application process for the Healthy Indiana Plan
Step 1: Determine your eligibility for the plan.
Step 2: Get an application from Howard Regional Health System by calling 765-453-8422 or download and print an application from www.hip.in.gov.
Step 3: Complete all sections of the application truthfully and as completely as possible in blue or black ink.
Step 4: Sign the application where indicated on page 4.
Step 5: Gather copies of the materials listed on page 1 of the Information to Get You Started handout included with application. These materials must accompany your application.
Step 6: Submit the application to the Healthy Indiana Plan. You can submit the application in any of the following ways:
- Call 765-453-8422 to schedule an appointment. Then bring your application and materials from Step 4 to Howard Regional Health System, Intake Counselor - HIP, 3500 South Lafountain Street, Kokomo, Indiana 46902.

Step 7: Cooperate with the Healthy Indiana Plan on requests for further information or interviews. HIP will contact you by phone or U.S. Mail if additional information or documentation is needed for your application. HIP requests that you respond quickly to any requests for additional information so your application can continue through the process.
