Pre-register at Our Hospitals

Thank you for choosing Howard Regional Health System.

We know your time is valuable. By pre-registering you may save up to 20 minutes of wait time.

Please submit your pre-registration form at least two days prior to your scheduled visit. This allows the staff adequate time to verify and pre-certify your insurance (if applicable).

You will receive confirmation that your information was received if you provide a valid email address.

For Main Campus: Please call Centralized Scheduling if you have questions, 765-864-8929 or toll-free at 866-864-8929. Our hours are Monday through Friday 8:00 a.m. until 5:00 p.m. If you need to schedule an exam on a weekend, please contact our main switchboard at 765-453-0702 and the operator will connect you to the direct department for all your scheduling needs.

On the day of your procedure, please come to Admissions on the first floor of the Main Campus hospital and bring the following:

Driver's license or picture ID
Insurance card(s)
Physician's order (a signed order from your doctor with an ICD9 code or written diagnosis)

For West Campus: If you have questions, please call Monday through Friday from 8:00 a.m. until 4:30 p.m. Sleep Center can be reached at 765-454-4589 and Outpatient Registration at 765-454-4566.

On the day of your appointment, please come to the back of the hospital and enter the Outpatient Services, Sleep Center entrance. Please bring the following:

Driver's license or picture ID
Insurance card(s)
Physician's order (a signed order from your doctor with an ICD9 code or written diagnosis)

After completing the form, click on submit. If information is incomplete or needs clarifications, the staff may need to complete the registration by telephone or when you arrive.
